Nurse Case Manager


Job Description:

Our valued client in Beautiful Hawaii is hiring a Nurse Case Manager who would be responsible for assuring access and provision of quality medical care to select participants/residents receiving services. S/He will provide participants/residents with necessary services to attain and maintain optimal health status.


Responsibilities:


  • Ensures that the rights of the participants/residents are protected and that they are protected from abuse and neglect and dangerous or hazardous conditions following established reporting procedures. Ensures confidentiality of all participants/residents' protected health information and records from unauthorized access or dissemination

  • Ensures adherence to the normalization principle in the least restrictive environment

  • Ensures program and facility compliance on all applicable standards, policies, and regulations

  • Ensures that the nursing standard operating policies and procedures are current and updated as needed, and staff members are trained on said policies and procedures

  • Provides basic First Aid/CPR to participants, residents, staff, and others, when needed

  • Provides health care explanations and guidance to families and guardians

  • Participates in department quality assurance program

  • Reports medication errors and incidents immediately, and initiates GERs, as needed

  • Maintains accurate and complete documentation in resident medical record by all individuals providing health care; provides reports as needed in regarding medical concerns

  • Observes and assesses resident's condition through monthly and annual exams and provides direct care whenever necessary

  • Develops, implements, evaluates, and updates resident's Health Maintenance Plan

  • Communicates with other disciplines regarding resident?s specific needs. Participation in Individualized Program Plan conferences, and documentation of resident?s progress in the T-logs

  • Initiates and receives physicians? orders and ensures accurate implementation of orders

  • Coordinates provision of care by the primary care physician, specialists, other health care personnel in a timely manner and attends resident?s exams, as indicated

  • Directs and reviews medication administration practices by verbal and written communication with Home Managers, Program Manager, and staff, and by noting and reviewing medication administration records at least quarterly

  • Coordinates resident-related pharmacy issues to assure medication orders are filled accurately and received on time

  • Performs and documents site reviews of resident health, safety, medication administration, and infection control as scheduled and as needed


Education Requirements:


  • Bachelor's degree in the field of nursing from an accredited college or university; current Hawaii state license as a registered nurse


Desired Background/Skills:


  • Smart phone required

  • State of Hawaii driver?s license and Visions certification

  • One year experience as a registered nurse

  • Strong verbal, written and interpersonal skills; positive, helpful, and patient; good judgment

  • Ability to organize, prioritize work and meet deadlines

  • Proficiency in computer software programs related to job duties

  • Ability to proficiently operate standard office machines, standard household appliances and assistive devices including Hoyer lift, wheelchair, walker and gait belt

  • Ability to intermittently lift, push and pull up to 50 lbs., with minimal assistance

  • Current work clearances which may include, but are not limited to, PE, initial 2-step PPD, annual PPD, CPR, First Aid, criminal history, APS & CAN registries, FBI fingerprinting and a driver?s abstract with an acceptable level of infractions. Hepatitis B

  • vaccination optional but, recommended.
